Your practice must sell a minimum of 30 pairs of qualified Shamir lenses per month to accrue points.

Why am I required to Enroll? All practices, new to the program, are required to enroll to properly set-up their Account with the required information: practice name, contacts, point recipients and laboratory along with a W-9 for tax purposes.
